Improving Community Health Using the Medical Home Model at Safety Net Hospitals

Safety net hospitals, often in collaboration with local health departments, are increasingly using medical homes to provide greater access to care. Researchers at the National Association of Public Hospitals and Health Systems conducted a study to determine the implications of creating a medical home in a safety net hospital environment. The findings suggest that medical homes increase vulnerable patient populations’ access to primary care and disease management and prevention programs, while simultaneously reducing over-use of the emergency department. Therefore, the study indicates that the adoption of the medical home model can be a cost-effective, efficient approach to improve community health.
