PPHR Executive Summary Tip Sheets

These one-page PDF was developed by NACCHO and approved by the PPHR Workgroup. The documents describe the information that should be included in the Executive Summary element of a PPHR application and suggests a format for the Executive Summary Document.

The first document is intended for local and county LHDs, and the second is intended for regional applicants. This information is also included at the bottom of the current versions of the standard and regional PPHR criteria.
