Prerequisite: Greene County Combined Health District Community Health Improvement Plan (2009)

In 2009, the Greene County Combined Health District, in partnership with 40 different community leaders, developed this community health improvement plan (CHIP), one of the three prerequisites for national accreditation and interested in developing a CHIP.  This 25-page document outlines nine key community health goals along with established outcomes to measure success, lead community agencies responsible for each goal, and identified strategies to accomplish the goals. This resource is useful for those looking to develop a CHIP in preparation for national accreditation.
