Description:

The Health Problem Analysis worksheet allows users to identify high level causes to a community-wide health problem and may serve as a precursor to initiating quality improvement (QI) efforts. This tool guides users through multiple layers of a health problem, uncovering direct and indirect contributing factors. Results of this worksheet can be taken to conduct a deeper root cause analysis and eventually identify potential interventions with measurements for improvement.
