Able To Be Healthy Guide


This tool will educate readers about health risks, health management, and available resources to empower people with disabilities to improve their health.  The Central Connecticut Health District developed, designed, and distributed the Able to Be Healthy Guide to residents of four towns.  

  • The guide includes the following:
    • National, state, and local fitness resources;
    • Adaptive resource lists;
    • Healthy shopping lists;
    • Easy to prepare, healthy meals;
    • Means of reducing secondary health conditions; and
    • Overview of area fitness facilities.
    This tool can be modified to include resources and facilities in your area.
This guide was developed as part of NACCHO's Health and Disability technical assistance project.
