ICS Roster Template

This roster template allows LHDs ot list the primary, secondary, and tertiary staff to cover the command and general leadership roles during a response based on the National Incident Management System (NIMS). The roster also integrates the Incident Command System (ICS) trainings for each staff member.

This document was submitted for Goal 1 of a Project Public Health Ready application and can be used as an example by other agencies.
