Schenectady County Strategic Alliance for Health Implementation Guide


The purpose of this implementation guide is to provide a case example for the Strategic Alliance for Health initiative funded by the Centers for Disease Control and Prevention’s Healthy Communities Program. The guide offers communities a “how to” handbook for implementing a policy, systems and environmental change strategy for chronic disease prevention. 

Persons with intellectual and developmental disabilities face an increased risk for chronic diseases such as heart disease, obesity and diabetes, compared to the general population. Although these higher risks are present, few community-based interventions targeting those with intellectual and developmental disabilities exist. However, there are steps that can be taken to lower the risks for developing chronic diseases and obesity, such as improving access to and consumption of healthier foods.

This Know, Grow, and Eat Your Vegetables Program was designated as a Model Practice by NACCHO in 2012.  

This Implementation Guide provides an overview of how Schenectady ARC, in collaboration with the Schenectady County Strategic Alliance for Health, has improved access to and consumption of fresh produce for persons with intellectual and developmental disabilities.
