Coordination with Indian Tribes-Survey Template

This four-page template for identifying the work that has and continues to be done between public health and Indian tribes was created by the Idaho Department of Health and used by Southwest District Health, for Goal 1 of Project Public Health Ready.

This survey template attempts to identify how Indian tribal governments participate in local preparedness planning and implementation by asking questions that have been formulated to:

Identify areas that local public health preparedness needs to focus in the future,
Prevent duplication of efforts, and
Identify improvements in coordination efforts that can be reported back to CDC.
