This 38 page PDF is an example Training Needs Assessment & Training Plan submitted to Project Public Health Ready based on the 2004 criteria requirements for Goal 2: Workforce Competency, by the Stanislaus County Health Service Agency.
The training needs assessment has a comprehensive introduction and the assessment results and implications are very well illustrated with detailed graphics to communicate key data findings. The data analysis is good because it summarizes what are the priorities for future trainings, followed by comprehensive short-term and long-term training plans with training matrices. The training plan is strong because there is a section that shows how the staff participating in the training are going to demonstrate competency after the training, which staff job titles are schedule to participate in the trainings and how many of the individual who hold each title will participate.
