The Health and Disability Project
Demonstration Sites
The National Association of County and City Health Officials (NACCHO), with guidance and funding from the National Center on Birth Defects and Developmental Disabilities of the Centers for Disease Control and Prevention (CDC) identified and awarded five demonstration sites to improve their capacities in addressing the health and well-being of individuals with disabilities.
Technical Assistance Project (TA Sites)
NACCHO also offered technical assistance support to local health departments to improve their capacity to develop or enhance health promotion programs for people living with disabilities through coordination of existing health department and community services. The local health departments that received technical assistance through this program are called TA sites.
Below are the pairings of demonstration sites and the local health departments that received technical assistance (TA sites). Click on any site for more information.
