Tool- Two Models of CHA/CHIP Roles

These customizable tools are two proposed role and responsibility assignment descriptions for each of the eight participating LHDs and their partners in the East Central Kansas Public Health Coalition (KS). This is a coalition of eight LHDs who have formally shared services who are jointly conducting a CHA and developing a CHIP across all eight LHD jurisdictions.
