Apply for PHAB Accreditation

Once an LHD has determined it is feasible to apply and completed all the preliminary accreditation preparation efforts listed throughout this website, it is ready to submit a letter of intent to PHAB and begin the application process. Visit PHAB's website for information about the steps in the application process. Some available resources are below.

If your health department has applied for PHAB accreditation, let NACCHO know how the process is going and how we can help. We can provide technical assistance or link you to resources that can assist in the process. Send an e-mail to accreditprep@naccho.org with your contact information and relevant details.
Accreditation Coordinators Learning Community
Accreditation coordinators from local health departments that have applied to PHAB are invited to join NACCHO’s Accreditation Coordinators Learning Community. This group provides a space for accreditation coordinators to network with peers, share effective practices and lessons learned, and find answers to their questions.
