Project Overview: The City of Milford Health Department (CMHD) is located in Milford, CT, and aims to prevent disease and promote a healthier environment for all citizens through provision of quality services. CMHD focused on improving the department's capacity to conduct periodic community health assessments. CMHD staff and community stakeholders were trained on the importance of community health assessments and quality improvement to increase knowledge of assessment processes, and to brainstorm potential priority areas for the community.
