The examples below provide guidance to help local health departments (LHDs), non-profit hospitals, and other organizations use Healthy People 2020 (HP 2020) to conduct community health assessment and improvement planning processes.

In 2012 the Palm Beach County Health Department (FL) engaged the Health Council of Southeast Florida to facilitate a comprehensive, county-wide health assessment for Palm Beach County. St. Clair County Health Department
The St. Clair County Health Department and County Health Care Commission (IL) developed a comprehensive community health assessment of needs and a five-year community health plan in fulfillment of the Illinois Department of Public Health IPLAN Certification Requirements for LHDs. Professional Research Consultants created a Health Forecast for Hidalgo County (TX), an interactive tool designed to make community health needs assessment data widely available to the communities they reflect.
