Wisconsin Story from the Field


Hospitals, Public Health Creating Community Value: Practical Tools for Moving Into Implementation: Background on  Wisconsin (WI) communities are conducting the community health improvement process under the WI statutory requirement for health departments to develop a community health improvement plan and process (CHIPP) and an in depth look at two rural counties where hospitals and public health are collaborating in their CHIPP efforts.

  • The presenters describe the CHIPP work that has been happening in Wisconsin, including background information on the WI statutory requirement, how hospitals and health departments are collaborating, the CHIPP Infrastructure Improvement Project including the shared model of community health improvement, and tools and resource for implementation.   • The presenters provide a real life example of CHIPP efforts in two rural counties, Ashland and Bayfield Counties, WI, through a partnership between Ashland County Health and Human Services Department, Bayfield County Health Department and Memorial Medical Center.
